ERM-14
ERM-14 is NCCI's standard form for reporting a change in ownership, so the rating bureau can apply the combinability rules correctly and decide whether a prior entity's experience modification rate should follow into the new or restructured company. It gets filed when a contracting business is sold, merged, or otherwise changes ownership structure. If the form isn't filed, or is filed with incomplete information, the wrong claims history can end up carried into your current EMR (experience modification rate), or a history that should have transferred doesn't. This is one of the more technical corrections to chase down, since it usually involves your agent, your carrier, and sometimes NCCI directly, but it's worth pursuing specifically if your company has changed ownership structure and your mod doesn't seem to reflect your actual claims record. States with their own independent rating bureau may use an equivalent form rather than ERM-14 itself, though the underlying purpose, reporting an ownership change so combinability is applied correctly, is the same.
